This agreement between the undersigned and Greenery Collective is for the purpose of promoting a community of individuals who share a common interest in health and well-being. Each member of the Collective must each agree to the following terms and provide the required information set forth below.
Each member must meet the following requirements to join the Collective.
- You must be at least eighteen (18) years of age;
- You must have a current California Medical Marijuana ID Card and/or a physician recommendation for medical cannabis;
- Only approved individuals may join the Collective;
- Members must always present a current California Medical Marijuana ID Card and/or a physician’s recommendation for medical marijuana;
- Members are limited to one collective visit per day and in the company of a member of the Patient Comfort Delivery Collective management team; and
- Members agree to never sell or otherwise distribute medication obtained by the Collective.
Violation of these rules will be cause for immediate termination of an individual’s membership.
To join, each person must provide proof of valid ID including either a valid driver’s license, California ID, or Passport. The person shall also provide a copy of a valid Medical Marijuana ID card or physician recommendation. By registering for an account, you authorize the Collective to confirm your recommendation for the medical use of marijuana or the Medical Marijuana ID Card and this document shall constitute your written release per Health Insurance portability and Accountability Act (HIPAA).
Membership shall commence upon obtaining the required identification and verification. Each person’s membership shall expire at the termination of his or her recommendation or Medical Marijuana ID Card. Membership shall renew upon proof of new recommendation and/or issuance of a new card.
The Collective reserves the right to deny membership to any applicant. Applications are considered on case by case. The Collective reserves the right to terminate membership for any violations of the terms of this agreement with no warnings or second chances.
Members acknowledge that at the time of any delivery, the Member must be present with proper personal identification current California Medical Marijuana ID Card and/or a physician recommendation for medical cannabis.
Members understand that cannabis, while being a well-known effective therapeutic agent, is still regarded as illegal under federal law, according to the federal government. Therefore, all members acknowledge that they are committing an act of collective federal civil resistance.
I have read and understand this Membership Agreement and fully understand the terms thereto. I understand that my membership may be denied or revoked upon a violation of any of the policies stated herein.
